fix(server): native session replacement fails honestly when the provider reuses its session - #27517
When a resumed native session can't take an incoming handoff (resume failed, uncertain history delivery, or an exhausted context window, the recovery added in #25610), turn start asks the provider for a fresh native session and records a replacement handoff. Two safeguards are missing from that path:
A provider can return its existing session. Muse does this today when asked for a fresh one. Main records a replacement handoff and transfer anyway, then fails with a misleading "shorten the request" error. When the trigger is a failed resume, the automatic retry finds the reused session already marked loaded, skips the resume, and starts the turn on it with no error and no recovery summary.
Replacement events are written unconditionally. A run that stops while the replacement session is being created still gets a recovery handoff and transfer recorded against it.
Change
In the replacement path in ProviderTurnStartService:
After the forced ensureThread, a replacement whose native id equals the original fails the start straight away, through the existing settleStartFailure route, with a ProviderAdapterProtocolError: "the provider returned its existing session when asked for a fresh one, so the conversation context could not be moved to a new session". It is not retried, because the retry would silently continue on the reused session. A replacement with no native id still passes, as on main.
Replacement handoff and transfer events are written with writeIfRunCurrent against the starting attempt. The local handoff list and fallback flag change only after a committed write; otherwise the start stops.
Both apply to all three replacement reasons. Nothing else in #25610's recovery changes. Ordinary load errors still retry as before.
Scope and approval
This is a follow-up to #25610 for the two gaps above. Both safeguards came from #24579, which #25610 superseded; that PR's discussion compares the two. Muse support for a genuinely fresh session is not included; it needs a live Muse account to verify.
Verification
New tests in testkit/ProviderSwitch.integration.test.ts, with no sleeps:
Reused session on an exhausted window: the run fails with the honest message, no replacement handoff or transfer is persisted, and no provider turn starts for it.
Reused session after a failed resume: the run fails on the first attempt with the same message and no provider turn starts. On the first commit's code this test fails with expected 'completed' to equal 'failed' (the silent retry).
Run stops during replacement: no replacement records are written, the provider thread keeps its native id, and the next turn completes on the original session.
All three fail on main's original ProviderTurnStartService.ts.
vp test run on ProviderSwitch.integration.test.ts, ContextHandoffBudget.test.ts, ProviderTurnStartService.test.ts and SelectionRestart.integration.test.ts: 4 files, 105/105 passed.
vp exec tsc --noEmit -p . in apps/server, plus vp lint and vp fmt --check on the changed files: clean.
Not checked: live providers. The stop test sets the run state with a direct event write, because a real run.interrupt cancels the start effect before it reaches this code, so the guard covers the narrow window around that cancel and any other state change.

…der reuses its session
When a resumed native session cannot take an incoming handoff (resume
failure, uncertain history delivery, or an exhausted context window), turn
start forces a fresh native session and records a replacement handoff. Two
safeguards were missing from that path:
- A provider can answer forceNewNativeThread with its existing session (Muse
does). The replacement was recorded anyway, and the turn then failed with a
misleading "shorten the request" error. The replacement now fails before any
bookkeeping with a protocol error saying the provider returned its existing
session.
- The replacement events were written unconditionally, so a run that stopped
while the replacement session was being created still got a recovery handoff
and transfer. They are now written with writeIfRunCurrent against the
starting attempt, and start stops if the write does not commit.

…t retry
When a resume fails and the forced replacement comes back with the original
native id, the protocol error was propagated for an automatic retry. The
session manager has already marked that session loaded, so the retry skipped
the resume, found the budget fitting, and started the turn with neither the
error nor the recovery summary.
The identity check now runs after the thread load and settles the run as
failed straight away, whatever the retry budget, so the user sees the honest
error and no provider turn starts.

Macroscope's review found this PR approvable — Session recovery now reports an explicit failure when a provider reuses its existing session, and records replacement events only for the current starting attempt. The change is confined to recovery safeguards and leaves ordinary turn starts unchanged.

Walkthrough
Fresh-session recovery now fails if the provider returns the previous native session. Recovery handoff and fallback state are updated only after a conditional write commits for the current starting attempt. Integration tests cover both cases and an interruption during replacement creation.
Startup fails with a provider protocol error when a fresh-session request returns the existing native session. Integration tests cover context-handoff and resume-fallback replacements, with no replacement records or additional provider turn.
Startup returns without updating recovery state when the conditional write does not commit for the current attempt. An interruption test checks that the next run resumes the original native session.
Recovery can start a provider turn on the session it was meant to replace. Compare against the resumed session before merging.

i ran github/main (5fe18d79ac) first in a separate worktree, then the exact pr head, with one dev server at a time and isolated state. playwright from the repo drove /usr/bin/google-chrome. i created a project pointing at a real git repo and conversations through the web app, using real codex gpt-6.1-sol turns through the local proxy.
on both versions, initial creation and ordinary continuation returned the remembered marker. with the server stopped, i moved only the test conversation's native rollout file into backup, restarted, and sent another message through the ui. both logged resume_failed, created a different native id, injected the full-thread recovery handoff, and returned the original marker.
on the pr, read-only inspection confirmed one injected full_thread_summary and one resolved provider_resume_fallback for that recovery. i also stopped a real running codex tool call through the ui, then sent another message: the run was interrupted, the next turn completed with the marker, and no extra replacement records appeared.
blacksmith-check passed the existing provider-switch, handoff-budget, turn-start, and selection-restart checks: 4 files, 105 tests. existing ci reports 30 successful checks.
Not verified
the original reused-session bug was not reproduced on main. codex returned a genuinely fresh id, so the new same-id rejection and its no-retry behavior remain unverified end to end. muse is unavailable here. the new conditional write's successful commit path ran, but its rejection after a stopped starting attempt was not deterministically exercised; ordinary stop cancels the start effect. the existing automated cases passed, but they do not replace live proof of those two guards.
native ios/android, electron shell, macos/windows, other providers, and remote/relay/tunnel connections were not exercised. this is not complete live verification of the two new defensive guards.
