Add email verification inputs to minFraud

Builds on #6 (billing phone verification inputs); merge that first.

Adds two optional inputs to the minFraud request email object: was_verification_successful (boolean) and verification_time (RFC 3339 date-time). They describe the most recent verification of the email address. The request example and the bundled spec are updated.
