HTML Reporter fixes and improvements

When running tests using run-workers, noticed something

only some items show the correct feature name perhaps from the same worker, others from other workers show Unknown Feature
Steps details from Unknown Feature item lost all steps info
test history chart looks a bit blurry, maybe increase the resolution for the chart improve UI/UX
add button to go to top of page to improve UI/UX
some HTML Reporter: are still visible when not using --verbose or --debug mode
